New York Space Grant is required to report statistical information for all student awardees to NASA. In addition, follow-up surveys are done periodically to track educational and employment outcomes for all awardees. Please use the link below to enter your information.

*Note that this is a Google form, however it is managed through an agreement with Cornell University, and all information collected will be stored in accordance with the Family Educational Rights and Privacy Act (FERPA).
